Larsen Johansson Disease “Jumpers Knee”
It is osteochondritis or tendinitis occurring at inferior pole of patella. It is caused by repetitive traction or overload as in sports.
Clinical features
a) Pain
b) Soft tissue swelling
c) Limp and difficulty in kneeling
d) Extension of knee against resistance elicits pain.
Blazina’s 4 stages
1. Pain with activity
2. Pain during and after activity
3. Pain with restriction of activity
4. Unremitting pain
(End stage with patellar fracture or disruption of extensor mechanism)
Radiograph shows blurring, irregularity and radioluscency . Often there is elongation of inferior pole of patella. Calcification, periosteal reaction is seen over anterior surface of patella giving rise to tooth sign.
Management
a. Conservative management usually works for earlier stages.
Conservative management is given with :
1. Analgesics
2. Proper stretching and warming up
3. Exercises
4. Cast or brace
b. Surgical treatment is required for stage 4.
1. Stress fracture is treated by internal fixation.
2. Curettage and repair is done for disruption of extensor mechanism.
